West Oʻahu Produce Prescription Program

Our West Oʻahu Produce Prescription Program provides eligible adults with a $150 monthly mākeke voucher for six months.

Participants can use the voucher to purchase fruits, vegetables, and poi at our two farmers’ market locations. During the program, participants also complete health assessments and surveys and attend community workshops. Patient health data is tracked to see how a person’s diet affects overall health.

Eligibility Requirements

  1. You must have a primary care provider at Waianae Coast Comprehensive Health Center (WCCHC).  

  2. You must live on the Leeward Coast of Oʻahu in one of the following zip codes: 96792, 96797, 96707, 96706. 

  3. You must have a nutrition-related chronic disease, such as diabetes or high blood pressure. 

  4. You must have Medicaid insurance or receive SNAP/EBT.  

What to expect during the program

  1. Once we verify your eligibility, our team will contact you to explain program requirements, register you into our patient dashboard, and complete a survey.

  2. We will set an appointment for you to come to our office to take health measurements.  

  3. At your appointment, you will receive your West Oʻahu Prescription Card. We will load $150 into your card for the first month. Use this card at one of our farmers’ markets to purchase eligible fruits, vegetables, and poi.

To stay enrolled in the program, you must: 

  • Attend four community workshops

  • Complete surveys 

  • Visit our office every three months to take health measurements

You must be referred by your WCCHC provider to enroll in the program. Contact your WCCHC provider today.

  • Participants can purchase fresh fruits, vegetables, and poi from our two market locations.

  • Bring your West Oʻahu Prescription Card to one of our farmers’ markets and use it to buy eligible fruits, vegetables and poi. Simply give your card to the vendor when you’re ready to make a purchase. If you have any questions while at the market, visit our team under the market info booth.

  • Funds are reloaded on the 5th or 15th of each month.

  • Funds expire at the end of each month. They do not carry over into the next month.

  • No, but you can apply for our other Food as Medicine programs.

  • Yes, space is limited. The program will close once all spots are filled. Those hoping to join the program should contact their WCCHC primary care provider.

Other programs

  • Our Mana Ku Produce Prescription Program provides eligible parents with a $100 monthly voucher to purchase fruits, vegetables, and poi at our farmers market.

  • Our Food Subscription Program provides eligible households who have been denied SNAP/EBT with a $250 monthly voucher to purchase fresh produce, poi, honey, and proteins at our farmers market.
